Although we encourage involvement with your physician we do not require a referral for your treatment. Referrals from your family physician may be needed to claim through your Extended Health Care Plan. We are a fee for service clinic. Services may be covered by Extended Health Care Plans, MVA Insurance plans, Veteran’s Affairs. Payment is required on the day of service. For fees on other services we offer please contact the clinic directly. Assessments and treatment by the Sports Medicine physicians are covered under by OHIP.


Appointments: Please check in upon your arrival. For your initial assessment you will be asked to come in at least 10 minutes early to complete our forms. Follow-up appointments need to be scheduled with our reception. Please discuss the frequency and duration of each appointment with your therapist and book accordingly.

Cancellation Policy, No Show appointments: Any appointments you make are your responsibility. As treatment times are reserved for your benefit, we require 24 hours notice of appointment cancellation unless exceptional circumstances arise. You will be billed directly a fee of $25.00 if sufficient notice is not provided. (Most insurance companies will not reimburse your for this fee) We thank you for choosing our team at D. Freer & Associates and we would be happy to assist you at any time with any clarification that is required. Your recovery is our main goal and we will make every attempt to help make the billing process as smooth as possible.

Payment: D. Freer & Associates is a private health care facility. We believe in offering one-on-one treatment with the most experienced and well qualified therapists. Medical care is an expensive commodity in our society and the cost is forever increasing, taking this very important and essential service out of reach of many Canadians. This was acknowledged in medicine and a National Medical Plan was set up (OHIP in Ontario) to subsidize the costs and make medical care accessible to all people. Physiotherapy outside a hospital setting, Athletic Therapy, Massage Therapy and Personal Trainers, however, although well deserving the same consideration, have not acquired similar status. At D. Freer & Associates payment for your assessment and treatments are due at the time the service is provided in the form of cash, cheque, Interac, Visa or MasterCard. Please note there will be a $25.00 service charge for any NSF items. Post-dated cheques are not a viable alternative. It is expected that in the near future, physiotherapy clinics will be able to bill your health plan directly (as pharmacies and dentists do) but this technology is not yet in place. It is your responsibility to keep track of your plan specifics, submissions for reimbursement and educate yourself regarding your extended health insurance coverage. If you have been involved in a motor vehicle accident (MVA), you are eligible for coverage through the Automobile Insurance Act (Bill 59). The therapist will submit a treatment plan for the cost of your proposed treatment to your auto insurance company. However, under the current legislation, your extended health plan must be used first. We will gladly answer any questions you may have in regards to bill 59 and billing guidelines for MVA clients.

Privacy Policy: At D. Freer & Associates we respect your right to privacy. We recognize that you have provided, and will, from time to time, provide us with personal information such as your name and contact information, as well as health information. In compliance with The Personal Information Protection and Electronic Documents Act (PIPEDA) which went into effect January 1, 2004 this office adheres to the following principles:

     Accountability: All our employees and associates are responsible for maintaining and protecting all patient information under our control.

     Identifying Purpose: In most cases, we ask for information only so the appropriate medical care can be provided. If there are other reasons for collecting information, the purpose for which information is collected will be clearly explained before or at the time of collection.

     Consent: Your request for care at our facility implies consent for our collection, use and disclosure of your personal information only for purposes related to your care. Other purposes, such as sending information to Hospitals, Physicians or Insurance Companies require your written consent. We will record, use or disclose personal information only with your knowledge and consent.

     Limiting Collection: We will limit the collection of your personal information to only those details necessary for providing you the best possible care.

     Limiting Use, Disclosure and Retention: Your information will only be used for the purposes for which it was collected unless additional consent has been obtained for when it is required or permitted by law. Our patient information is kept for a period of 10 years.

     Accuracy: Your information is maintained in as accurate, complete and current form as possible to fulfill the purposes for which it was collected. Feel free to speak with us if you have any concerns.

     Safeguard Patient Information: Patient information is protected by security safeguards appropriate to the sensitive nature of the information collected.

     Patient Access: You have the right to see your records. You may also obtain copies of your records – there is a fee for this service. Please speak to the reception desk for further information.
